Diaphragmatic Breathing for Pain Relief: Breaking the Cycle of Tightness and Pain

Whether we are treating knee pain, shoulder tension, or chronic back issues, the mechanics of how you breathe often dictate how quickly you recover.
The Mechanics of Inefficient Breathing
On the surface, breathing seems simple—it happens without conscious thought. However, the efficiency of that breath is what matters for long-term health. Ideally, breathing should be driven by the diaphragm, a muscle lining the inside of the ribcage.
For many patients, pain and stress lead to tight neck and back muscles. This tightness pulls the ribcage into an improperly elevated position, placing the diaphragm in an ineffective mechanical state. This creates a "tug-of-war" where the neck muscles are winning, leading to a cycle of further pain and restricted mobility.
Breaking the Cycle with Core Stability
To reverse this process, we must engage the deep core muscles—specifically the internal obliques and transverse abdominis. These muscles attach to the lower ribs and have the power to pull the ribcage back into its proper alignment, allowing the diaphragm to function correctly.
While many reach for sit-ups or crunches to "strengthen the core," these exercises often tighten the neck further. Instead, our Physical Therapy approach focuses on guided exhalation and gentle inhalation to reset the nervous system and restore ribcage mobility.
